Actually, from what I can gather, there was a plan afoot during Sylvester McCoy's tenure to reveal that the Time Lords had stopped breeding the old-fashioned way ages ago, and all current Time Lords were created in batches of 44 (I don't know why 44) on genetic looms. The Doctor, however, was Speshul, having been created as a clone of a contemporary of Rassilon's known as The Other. And Susan wasn't his grand-daughter, because, uh, looms.
Or something like that. It's all really, really confusing.
The episide where all of this was supposed to be revealed got scrapped at the last minute and then the show was cancelled. The scrapped script was eventually reworked into one of the New Adventures novels, though. (Lungbarrow.) The writer is said to have been doing a Ghormenghast riff.
Since the novels are only dubiously canonical and they've had Ten say he was a father, I think probably the loom business has been abandoned by the current creative team.
